Cumberland County Community Health Needs Assessment and Implementation Plans

The Cumberland County Department of Health has completed the 2024 Community Health Assessment and Community Health Improvement Plans.

The Community Health Assessment offers a detailed look at the community’s health, highlighting key concerns, strengths, and challenges. Developed with valuable input from local stakeholders, community partners, and residents, it identifies priority areas that need focused attention and resources.  The Community Health Improvement Plan serves as a strategic plan to address the health priorities identified in the Community Health Assessment. The plan includes actionable goals, measurable objectives, and partnerships with organizations across the public, private, and nonprofit sectors.
