You can download, complete, and print the COVID-19 Consent Form (see button below) and bring to the clinic site. Forms can also be found in the lobby of City Hall (640 E. Wood St.).
Items you will need to get vaccinated:
- Your completed and signed Consent Form
- Your health insurance card (if you have one)
- Your valid photo ID
- Your vaccination card (if you have received previous doses of the COVID-19 vaccine)
- Your guardian (if under 18 years of age)
We offer clinics at various locations in Vineland. We are no longer hosting clinics at the Former Amish Market. All clinic locations, dates, and times will be posted here and on our social media sites.
