HOW TO GET VACCINATED
You must download, complete, and print the COVID-19 Consent Form (see button below) and bring it to your appointment. Forms can be also be found at the Vineland Public Library (1058 E. Landis Avenue) and the Lobby of City Hall (640 E. Wood St.).
Items you will need to get vaccinated:
- Your completed and signed Consent Form
- Your health insurance card (if you have one)
- Your valid photo ID
- Your vaccination card (if getting a second dose or additional dose)
- Your guardian (if under 18 years of age)
Our clinic is located at the former Vineland Amish Marketplace (formerly Newberry’s) on the 600 block of E. Elmer Street (use parking lot entrance).
While walk-ins are welcome, appointments are preferred. To book an appointment, see the instructions below.
How to book an appointment
Click the vaccine brand of your choice (below) to see the next clinic date and times available. Select an appointment time, then fill out the required information to complete your appointment booking. You will receive a confirmation email. If you need to cancel, you can use the “edit appointment” option at the bottom of your confirmation email.
If a date is “unavailable” appointments have been taken. Please check back for updates. Please arrive no earlier than 10 minutes prior to your appointment time.
